ABSTRACT

ObjectiveTo analyze changes of the chemical composition in Euodiae Fructus before and after processing with Coptidis Rhizoma decoction, so as to provide scientific basis for elucidating the processing mechanism of this decoction pieces. MethodUltra-performance liquid chromatography-quadrupole-time-of-flight mass spectrometry (UPLC-Q-TOF/MS) was performed on a Titank C18 column (2.1 mm×100 mm, 1.8 μm), the mobile phase was 0.1% formic acid aqueous solution-acetonitrile for gradient elution, the column temperature was set at 40 ℃, the flow rate was 0.25 mL·min-1. Electrospray ionization (ESI) was used to scan in positive and negative ion modes, and the scanning range was m/z 50-1 250. The chemical constituents in Euodiae Fructus were identified before and after processing by reference substance comparison, database matching and literature reference, and MarkerView™ 1.2.1 software was used to normalize the obtained data, SIMCA-P 14.1 software was employed to perform principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA) on MS data of raw and processed products to screen the differential components before and after processing. ResultA total of 50 compounds were identified, including 48 kinds of stir-fried products with Coptidis Rhizoma decoction and 44 kinds of raw products. After processing, six compounds were added, including danshensu, noroxyhydrastinine, oxyberberine, 13-methylberberrubine, protopine and canadine. However, two kinds of compounds, including (S)-7-hydroxysecorutaecarpine and wuchuyuamide Ⅱ, were not detected after processing. In general, after processing, the overall contents of phenolic acids and flavonoids decreased significantly, the overall content of limonoids increased, and the overall content of alkaloids did not decrease insignificantly. The results of PCA and OPLS-DA showed that there were significant differences in the composition and content of the chemical components of Euodiae Fructus before and after processing, and a total of 12 variables such as quercetin, dihydrorutaecarpine and dehydroevodiamine were obtained by screening. ConclusionEuodiae Fructus stir-fried with Coptidis Rhizoma decoction mainly contains phenolic acids, flavonoids, limonoids and alkaloids. The composition and content of the chemical components have some changes before and after processing. The addition of processing excipients and hot water immersion are the main reasons for the difference, which can provide experimental basis for interpretation of the processing mechanism of this characteristic processed products of Euodiae Fructus.

ABSTRACT

ObjectiveBy comparing the composition and content changes of the volatile components in Atractylodis Rhizoma before and after processing with rice-washed water, the effect of rice-washed water processing on volatile components in Atractylodis Rhizoma was investigated. MethodHeadspace-gas chromatography-mass spectrometry (HS-GC-MS) was used to detect the volatile components in rhizomes of Atractylodes chinensis and A. lancea, and their processed products of rice-washed water. Chromatographic conditions were programmed temperature (starting temperature of 50 ℃ for 2 min, rising to 120 ℃ with the speed of 10 ℃·min-1, then rising to 170 ℃ at 2.5 ℃·min-1, and rising to 240 ℃ at 10 ℃·min-1 for 3 min), the inlet temperature was 280 ℃, the split ratio was 10∶1, and the solvent delay time was 3 min. The conditions of mass spectrometry were electron bombardment ionization (EI) with ionization temperature at 230 ℃ and detection range of m/z 20-650. Then the relative content of each component was determined by the peak area normalization method. SIMCA 14.1 software was used to perform unsupervised principal component analysis (PCA) and supervised orthogonal partial least squares-discriminant analysis (OPLS-DA) on each sample data, the differential components of Atractylodis Rhizoma and its processed products were screened by the principle of variable importance in the projection (VIP) value>1. ResultA total of 60 components were identified, among which 40 were rhizomes of A. chinensis and 38 were its processed products, 46 were rhizomes of A. lancea and 47 were its processed products. PCA and OPLS-DA showed that the 4 kinds of Atractylodis Rhizoma samples were clustered into one category respectively, indicating that the volatile components of the two kinds of Atractylodis Rhizoma were significantly changed after processing with rice-washed water, and there were also significant differences in the volatile components of rhizomes of A. lancea and A. chinensis. The compound composition of Atractylodis Rhizoma and its processed products was basically the same, but the content of the compounds was significantly different. The differential components were mainly concentrated in monoterpenoids and sesquiterpenoids, and the content of monoterpenoids mostly showed a decreasing trend. ConclusionAfter processing with rice-washed water, the contents of volatile components in rhizomes of A. lancea and A. chinensis are significantly changed, and pinene, 3-carene, p-cymene, ocimene, terpinolene, atractylon, acetic acid and furfural can be used as difference markers before and after processing.

ObjectiveTo identify the chemical constituents of Alismatis Rhizoma before and after processing with salt-water by ultra-high performance liquid chromatography-quadrupole-time-of-flight mass spectrometry (UPLC-Q-TOF-MS), and to investigate the changes of terpenoids in Alismatis Rhizoma before and after processing with salt-water. MethodUPLC-Q-TOF-MS was used to detect with 0.1% formic acid aqueous solution (A)-acetonitrile (B)as mobile phase for gradient elution (0-0.01 min, 20%B; 0.01-5 min, 20%-40%B; 5-40 min, 40%-95%B; 40-42 min, 95%B; 42-42.1 min, 95%-20%B; 42.1-45 min, 20%B), electrospray ionization (ESI) was selected for collection and detection in positive ion mode with the scanning range of m/z 100-1 250 and ion source temperature at 500 ℃. The data were analyzed by PeakView 1.2.0.3, the components were identified according to the primary and secondary MS data, and combined with the reference substance and literature. After normalized treatment by MarkerView 1.2.1, the MS data were analyzed by principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA), and then the differential components before and after processing were screened. The content changes of differential components were analyzed according to the relative peak area. ResultA total of 30 components were identified under positive ion mode, including 28 prototerpene triterpenes and 2 sesquiterpenes. The results of PCA and OPLS-DA showed that there were significant differences in components from Alismatis Rhizoma before and after processing with salt-water, and 10 differential components (alisol B 23-acetate, alisol I, alismol, 11-deoxy-alisol B 23-acetate, alisol B, alisol C, 11-deoxy-alisol B, alisol G, 11-deoxy-alisol C and alisol A) were screened, and the contents of alisol G and alisol A decreased significantly after processing. ConclusionUPLC-Q-TOF-MS can comprehensively and accurately identify the chemical constituents in raw and salt-processed products of Alismatis Rhizoma. It takes a great difference in the contents of chemical constituents before and after processing, and the difference of substituents is the main reason for this differences, which can provide reference for determining the material basis of efficacy changes of Alismatis Rhizoma before and after processing with salt-water.

ABSTRACT

Objective:To summarize and review a Chinese family with cerebrotendinous xanthomatosis (CTX) so as to improve understanding of the disease.Methods:The proband was admitted to the Department of Neurology, the Second Hospital of Hebei Medical University on May 30, 2019. The medical history, neuro-imaging, pathology, CYP27A1 gene of the proband and CYP27A1 gene of her family were analyzed. Clinical features of similar cases from published literatures were retrieved and systematically summarized.Results:The proband was a 39-year-old female who was admitted to the Second Hospital of Hebei Medical University due to weakness of both lower limbs lasted for more than five years and aggravated for one year with speech slurred. The proband manifested with mental retardation, bilateral pyramidal tract impairment and cerebellar lesions, and had cholesterol crystal in xanthomas and compound heterozygous mutations of c.435G>A and c.1263+1G>A in CYP27A1 gene. The proband′s sister had the same mutation as the proband′s. The proband′s mother was the carrier of c.435G>A mutation, and father was the carrier of c.1263+1G>A mutation. Seventeen related cases concerning CTX with detailed clinical data were searched with major domestic databases. Combined with this case, clinical features with the frequency more than 50% were pyramidal sign, mental decline, ataxia, dysarthria, achilles tendon neoplasm, cataract, high arch foot.Conclusions:The onset of CTX is hidden, which can be diagnosed by its pathology and CYP27A1 gene detection. The possibility of CTX should be considered when there are unexplained clinical manifestations in common diseases such as pyramidal sign, mental decline, ataxia, dysarthria, achilles tendon neoplasm, cataract, high arch foot and so on.

ABSTRACT

Objective:To analyze the expression and role of activated transcription factor 3 (ATF3) in breast cancer.Methods:The Cancer Genome Atlas\Gene Expression Omnibus (TCGA\GEO) database information was queried, the expression level of ATF3 in breast cancer was analyzed by bioinformatics method and the survival curve was drawn. the parallel function and pathway enrichment analysis of ATF3 gene interaction network were constructed.Results:The level of ATF3 mRNA in breast cancer was significantly lower than that in normal breast tissue, but the expression level of ATF3 was significantly increased after chemotherapy. Survival analysis found that after systematic treatment, the survival period of breast cancer patients in the ATF3 high expression group was significantly prolonged, and the difference was statistically significant ( P<0.05). Enrichment analysis revealed that breast cancer-related pathways concentrate in Estrogens signaling pathway, mitogen-activated protein kinase kinases (MAPK) signaling pathway, phosphatidylinositol 3-kinase-protein kinase B (PI3K-Akt) signaling pathway, Wnt signaling pathway, Breast cancer signaling pathway, and Cell cycle signaling pathway. Conclusions:ATF3 is involved in regulating the proliferation of breast cancer cells. After systematic treatment, the high expression of ATF3 indicates a better outcome.

ABSTRACT

Objective@#Primary liver cancer is a kind of gastrointestinal malignant tumor with high morbidity and mortality. Early diagnosis is difficult, with postoperative high recurrence rate and high metastasis rate, poor prognosis, so it is particularly important to better understand the occurrence and development of liver cancer from the gene level, so as to provide theoretical basis for gene therapy or targeted drug therapy in the future.@*Methods@#TCGA database for liver cancer gene transcriptome data information and clinical phenotype information was downloaded, and R language edgeR package was used to standardize the transcriptome data. The log FC > 1, FDR < 0.01 gene was set to be expressing differences gene, and then the weighted correlation network analysis (WGCNA) methods was used to analyze the relationship between liver cancer stage (stage) and the differentially expressed genes to look for hub genes.@*Results@#Four hub genes related to tumor staging were identified (TPX2, HJURP, KIAA1524, SGO2). Kaplan-meier survival curve was drawn and it was found that their expression level was significantly correlated with patient survival time, and high expression level was an independent prognostic factor.@*Conclusions@#WGCNA method is used to mine the TCGA database information and find the hub genes related to tumor staging. The relationship between TPX2, HJURP, KIAA1524 and liver cancer has been reported in the literature, except SGO2. This study will provide important theoretical basis for the follow-up study of liver cancer.

ABSTRACT

Objective To investigate the effect of laparoscopic total extraperitoneal tension-free hernia repair (TEP) in the treatment of inguinal hernia with absorbable biological mesh.Methods A retrospective analysis of the clinical data of 19 patients with inguinal hernia with biological mesh was performed from September 2015 to September 2016 in Shengjing Hospital of China Medical University.The average operative time,average incision pain time,average postoperative venting time,average hospital stay,average operative cost,and long-term chronic pain,postoperative complications,and recurrence rate were observed.Results All the 19 patients were successfully operated.The average operation time was (35.0 ± 4.0) min,the average incision pain time was (1.5 ± 0.6) d,and the average postoperative venting time was (1.5 ± 1.2) d.The average hospital stay was (4.5 ±0.9) d,the average surgery cost was (23 534.6 ± 1 259.9) yuan.Two patients developed swelling of the scrotum after surgery,and one patient developed postoperative fat liquefaction.All patients were followed up for 1 year by telephone,with no recurrence,no serious complications,and chronic pain discomfort.Conclusions The application of absorbable biopsy TEP is a good and reliable way to treat inguinal hernia.Postoperative patients have mild pain and low complication rate.There is no recurrence in recent follow-up,and the short-term effect is satisfactory.It is a reliable method for the treatment of inguinal hernia.The cost is high,and can be selected according to age and individual needs.

ABSTRACT

Objective To explore the experience and efficacy of laparoscopic diagnosis and treatment of internal gallbladder fistula (IGF). Methods The clinical data of 24 cases of IGF treated by laparoscopic exploration/surgery were retrospectively analyzed in Shengjing Hospital of China Medical University from January 2012 to January 2018. Results All the 24 patients had underwent laparoscopic exploration and were confirmed as IGF. Among these patients, 16 cases had cholecystoduodenal fistula, 3 cases had cholecystocolic fistula, 3 cases had cholecystogastric fistula, and 2 cases had cholecystocholedo fistula (Mirizzi syndrome). All patients underwent cholecystectomy, and patients with common bile duct stones underwent common bile duct exploration and T tube drainage. Treatment of internal fistula including:duodenal fistula repair (16 cases), colon fistula repair (3 cases), gastric fistula repair (3 cases), duodenal fistula repair and gastrectomy (1 case), and Roux-en-Y cholangiojejunostomy (1 case). Eleven cases were performed laparoscopically (45.8%);13 cases were converted to open surgery (54.2%). Postoperatively, 2 patients experienced mild bile leakage and healed after active drainage for 2 months, and the rest recovered well. After 6 months of follow- up, no complications such as gastrointestinal fistula, obstruction, or stenosis occurred. Conclusions The clinical manifestations of gallbladder fistula generally lack specificity, so that preoperative diagnosis is difficult. IGF is hardly to self- heal, and surgical treatment is still the mainstay for treatment. For appropriate situation, experienced surgeons with the developed laparoscopic techniques are reliable treatment for IGF.

ABSTRACT

Objective To draw for character effective sports therapy for middle‐age patients with type 2 diabetes .Methods Choosing 16 middle‐aged patients with type 2 diabetes suitable for sport ,testing the VO2 max ,fasting plasma glucose and the differ‐ent time points after meals ,different intensity exercise corresponding heart rate to find out effective exercise intensity and blood glu‐cose peak time .To determine patients the best starting time ,carrying out effective intensity and time exercise 15 ,30 ,45 ,60 ,75 min before the peak time .Results (1)The blood glucose significantly reduces that 15 min low intensity and 5 min medium intensity were compared with pre‐movement(P0 .05) .(3) Glucose‐lowering effect was no difference of the movement at different time point before the peak ,but 15-45 min before the peak time was better .Conclusion The middle‐aged patients with type 2 diabetes should combine their own blood glucose peak time ,proceeding moderate intensity more than 15 min or low intensity more than 45 min ,they can achieve good hypoglycemic effect at 15-45 min before the peak time .

ABSTRACT

Objective To assess the efficacy of vancomycin flush solution for prevention of catheter-related infections in oncology patients. Methods Randomized controlled trials(RCTs) were searched and identified from Cochrane library, PubMed, EMBASE, CBM, CNKI and related journals were also scanned. In accordance with inclusion and exclusion criteria, we evaluated the quality of the included studies by Jadad scale and analyzed the data of catheter- related sepsis, catheter colonization, exit- site infection by RevMan 5.0. Results We included 5 randomized controlled trials (400 patients). Meta analysis showed that there were statistical differences between vancomycin- heparin mixture and heparin solution on the incidence of catheter-related sepsis, catheter colonization, but no difference was found on the incidence of exit-site infection. Conclusions The current evidences show that contrast with heparin flush solution, van comycin - heparin flush solution can significantly reduce the incidence, of catheter - related sepsis and catheter colonization. But vancomycin-heparin flush solution was more effective, and the results should be further confirmed in prevention of Gram-positive bacterial and fungal catheter-related infections or in non- neutropenic patients. Before draw the conclusions, further high-quality, large scale, double-blind randomized controlled trials are still needed.

ABSTRACT

OBJECTIVE:To evaluate the efficacy and toxicity of paclitaxel combined with cisplatin in the treatment of advanced non-small cell lung cancer(NSCLC) by weekly-,biweekly- and three-weekly-regimens.METHODS:180 patients who had been confirmed by pathology and cytology as having NSCLC(Ⅲ~Ⅳstage) were enrolled into the study and divided to three groups:Biweekly - regimen(n= 60):paclitaxel 80 mg?m~(-2) plus cisplatin 40 mg?m~2 ivgtt on day 1 and day 8 in every 21 days;Weekly- regimen(n= 60):paclitaxel 55 mg?m~(-2) plus cisplatin 30 mg?m z ivgtt on day 1,8,and 15 in every 28 days;Three - weekly regimen(n = 60):paclitaxel 160 mg?m~(-2) plus cisplatin 80 mg?m~(-2) ivgtt on day 1 in every 21 days.Serum concentrations of paclitaxel at 3,12,24 h after administration were determined,and the efficacy and toxicity after two- cycle treatment were evaluated.RESULTS:The overall response rates of weekly-,biweekly -and three weekly regimens were 43.1%,35.8%and 34.0%respectively,showing no statistical differences among groups,but the incidence of main toxicities of biweekly-regimen was lower as compared with the other regimens.CONCLUSION:Biweekly -regimen is optimal for the treatment of advanced NSCLC with mild toxicity,which deserves to be applied in clinical practice.
